GP health record

The information in your GP health record will depend on your individual GP practice. Depending on what your GP Practice have enabled you may be able to see your:

  • allergies
  • immunisations (vaccinations)
  • health conditions
  • test results such as blood tests
  • consultations and events such as appointments
  • your personal information (such as date of birth)
  • allergies and adverse reactions
  • medicines you have been prescribed (both in the past and ongoing)

If you would like to see more of your medical record than you currently have access to, please contact your GP Practice.

What your record shows

Depending on your GP practice, you will be able to see a summary view of your record. This will generally be a list of medicines, with their titles and when they were prescribed and any allergies you may have told your GP about.

Medicines may include repeat medicines (current) and any discontinued medicines you've been prescribed.

Access to more detailed records

If you only have access to your summary record, you'll see a message on your GP health record. The message will let you know that you can request access to more information.

To see more information in your record, such as test results and immunisations, contact your GP practice and request access to your "detailed coded record".

You can see test results carried out by your GP practice if you have access to your "detailed coded record".

Downloading documents from your detailed GP health record

It's not yet possible to download your whole record from your NHS account. For more detailed information about your record, contact your GP practice.

Sharing your GP health record

You will be able to select data that you'd like to share with others in the NHS Wales App. This feature will be added to the App in future releases. If your health board has chosen to use a third-party to provide some of your health information, you may be able to access this function through their site. 

The information in your GP health record is wrong

If information in your GP health record is incorrect, contact your GP practice. They will be able to update personal information in your record, such as your address. You can update the email and phone number held at your GP practice in the NHS Wales App. See Communications preferences for details about how to do this.

Note that the email and phone number held by your GP practice may not be the same one you have used to log into the NHS Wales App. If you change the email held at your GP practice it will not change the login details. You will need to go to NHS login to make any changes to your login details. 

If the whole record is not yours, you should contact your GP practice immediately.

There is missing information in your GP health record

Contact your GP practice if something is missing from your GP health record. It may be missing because you do not have access to all the information in your record. Ask them to give you access to your "detailed coded record".

COVID-19 information in your GP record

 Your GP health record cannot be used to prove your vaccine status when travelling abroad.

If you’ve had the coronavirus (COVID-19) vaccine, you may be able to see it in your GP health record.

However, this will depend on:

  • whether your GP has given you access to this information
  • where you received your vaccination
  • how the vaccination was recorded

 It will usually appear in your GP health record within 48 hours but may take longer if it needs to be added manually. If you cannot see your COVID-19 vaccine after 48 hours, make sure you have access to your "detailed coded record". 

Vaccination date

 The date that you received your vaccination may differ from the date in your records. This is because of the way GP systems record the vaccination information. The date will be when the information has been uploaded to the system, not necessarily the date you received it.

 This will not affect your COVID-19 Pass or the date you are called to receive your booster.
