If your GP practice has given access, you'll be able to see a summary view of your health record in the NHS Wales App. This will generally be a list of medicines with their titles, when they were prescribed, and any allergies you've told your GP about. Medicines may include repeat medicines you're currently taking and any discontinued medicines.
This is known as your summary care record (SCR).
If you can only see your summary care record, there will be a message saying you can request access to see more information. This is called your detailed coded record (DCR) and will show:
You can request access to your detailed coded record by contacting your GP practice. It's the practice's decision whether they approve this.
You can see test results carried out by your GP practice if you've been given access to your detailed coded record.
It's not yet possible to download the whole record from your NHS login. For more detailed information about your record, contact your GP practice.
You'll be able to select data you'd like to share with others, such as family and healthcare providers, in the NHS Wales App. This will be added to the App in the future. If your health board has chosen to use a third-party to provide some of your health information, you may be able to access this through their site.
If the information in your GP health record is incorrect, contact your GP practice. They'll be able to update the personal information in your record, such as your address. You can update the email and phone number held by your GP practice in the NHS Wales App. Communications preferences explains how to do this.
Note that the email and phone number held by your GP practice may not be the same one you've used to log into the NHS Wales App. If you change the email address held by your GP practice it will not change the login details. You'll need to go to NHS login to make any changes to your login details.
If the whole record is not yours, you should contact your GP practice immediately.
Contact your GP practice if something's missing from your GP health record that you think should be there.
