My Health Journal

My health journal allows you to create your own written journal entries within the NHS Wales App. You can write any information about yourself and your health, up to 4,000 characters in total length. This information is securely stored within the NHS Wales App and is only accessible by you. The information you provide within the journal is not sent to your GP Practice or other health and care professionals.

Accessing My health journal and creating an entry

  • Go to My health.
  • Select My health journal.
  • Select Add a new journal entry.
  • Input a title for your journal entry. (This should be meaningful to aid searching. For example, “Reminder to book diabetes test” rather than just “Reminder”)
  • Input the chosen Date of event.
  • Input the chosen Time of event (24-hour format – for example 22:00 for 10pm).
  • Use the Description box to compose your entry.
  • Select Save to save your journal entry.
  • You will see confirmation that your entry has been added.
  • Select Go to my journal to view your journal entries on My Health Timeline.

Note: Journal entries cannot be edited once saved.

My health journal is presented as clean text, you cannot insert an image or other non-text items. If you insert a link, it will render as text and would not operate as a hyperlink. 

Text from other sources can be copied into the journal, subject to your device’s capability. We are not responsible for accuracy or quality of content copied from external sources.

 

Example uses of the My health journal

  • Making notes whilst at a GP or other health and care appointment.
  • Create personal logs to track and monitor your health and wellbeing. For example: create an entry containing the date, time and pain severity of an arthritis flare up.
  • Set a future date as a reminder to yourself. For example, add a journal entry for six months away regarding booking blood tests so you will see it in your My health timeline as a reminder.

 

Using My health timeline to search journal entries

My health timeline can be used to search for past and future journal entries – see the My health timeline help page for further information.

Through My Health Timeline, you can also search journal titles by keyword. When composing your journal entries, you may then consider what words or phrases would be best suited for searching in the future. My health timeline does not allow you to search for words or phrases in a journal’s description, so creating descriptive and detailed journal titles will be more beneficial when searching.

 

Deleting a My Health Journal entry

  • Locate a journal entry on My health timeline.
  • Select the journal entry you want to delete.
  • Select Delete this journal entry.
  • Select Yes and then Submit to confirm deletion.
  • You will see a screen confirm deletion of the entry.

Note: Deleting a journal entry is permanent and cannot be undone.
